When a nurse is at your side, whether at the hospital or in your doctor's office, chances are, you will receive more of their time and attention.  Having a nurse with you elicits greater attention to detail from your healthcare providers, and with that comes greater satisfaction on your part, and an increase in your safety.

You don't have to go anywhere, I will come to you.  I can facilitate greater comfort, on your part, by meeting you at your home, or wherever you are comfortable.  If you prefer a phone conversation, or video chat, we can do that too.  I am available every day of the week.  Knowing that your own private nurse advocate is just a phone call away, can put you at ease.

  • Educate on diagnosis/procedures/treatment options/medications

Patient advocates have the time for you, to ensure that you have a full understanding of all of your health related conditions, medications, and treatments.  Are you always made aware of alternative treatment options? As your patient advocate I can help you learn everything you want to know, and I'm competent in researching multiple topics from reputable and trustworthy sources.  I then relay that information to you in a manner that matches your learning style and needs.

Do you have a new diagnosis? New pills? Are you considering a second opinion? Often, doctors will only offer treatments that they are familiar with, but wouldn't you like to know if there are additional options? There are multiple approaches to achieving wellness, and it may be worth knowing what is available to you.  As a patient you need to feel confident in your decision making when it comes to treatment options for new diagnoses, as well for medications.  Miscommunication in healthcare is a by-product of our fragmented system.  Doctors often do not consult with each other the way we think they do (or should).  A patient advocate is savvy to such issues, and will be by your side to protect you and provide you with educational materials to help you make informed decisions about your treatment options.

How many times have you left the doctor's office only to realize you forgot to ask that one important question? Perhaps you felt rushed. Did your doctor explain the potential side effects of your new medication? Do you assume that all of your doctors communicate with each other? I can help ensure a direct line of communication between you and your healthcare team.  I can take notes for you and ask appropriate questions to the medical team and if needed interpret that information in a way that makes sense to you.

A medical timeline and medication list shared with your physician can improve their understanding of your current and past health issues, and improve care.  I can organize your medical records and compile that information into a personalized timeline.  Physicians love having the ability to see a timeline like this.

Family Visit

Does your family need an unbiased party to help facilitate a smooth healthcare transition or mediate disagreements.  A patient advocate can sit with you, listen to your story, and create a plan that puts you or your loved one at the centre of the equation.  Maybe you are just not sure what to do after you are discharged from hospital.  Do you understand all of your instructions?  There are numerous possibilities of how I can help you.  Feel free to ask.

Do you need a care coordination meeting so your current care team can formulate a plan, but you are not sure how to go about doing this?  We can schedule care plan meetings, phone, or face-to-face visits with your health team and family.  Whether you are a family member or a patient yourself, I can coordinate and facilitate meetings to ensure that everyone is on the same page and your care is effective and safe.
